New Dog Authorization and Agreement Form
To be used for the cryogenic freezing of a stud's semen who is not currently stored at ICSB. This form can be used for an in-office appointment or at show, and also can be returned with Cryo/Epi Kits to ICSB when the samples are shipped back for freezing.
Repeat Authorization Form
To be used for the cryogenic freeing of a stud's semen for all collections after their first successful collection. This form can be used for appointment in office or at show, and also can be returned with Cryo/Epi kits to ICSB when the samples are shipped back for freezing.
Frozen Semen Release Authorization Form
To be used for the shipping of frozen canine semen stored at ICSB in OREGON to a bitch owner/veterinarian/storage facility.
The form can be completed separately by the semen owner, bitch owner, and cardholder; we can combine them upon receipt. As soon as we receive the form, we will confirm receipt and retrieve estimates for shipping and will contact the cardholder to confirm a shipping date and shipping speed.
A Release Form keeps the current semen owner as the legal owner of the semen and only requires one semen owner to sign off. They are releasing the semen for the desired breeding as listed on the release to a recipient/bitch owner. When a litter is produced, the semen owner will have to sign off on the litter registration, not the recipient/bitch owner.
This form is valid for 90 days upon receipt.
Fresh-Chilled Semen Shipment Authorization Form
To be used for the collection and shipment of fresh-chilled semen from ICSB to a bitch owner/veterinarian for insemination.
Each collection & shipment requires a new form signed by the stud AND bitch owner, even if the same breeding cycle.
Service Authorization Form
To be completed for authorization of specific services completed by ICSB as listed on the top of the form.
Canine Semen Evaluation Authorization Form
To be used to authorize ICSB to collect and evaluate semen with no freeze attempt.
Transfer of Ownership Form
To be used to transfer ownership of frozen canine semen stored at ICSB. This form must be signed and dated by an owner of the frozen semen.
A Transfer of Ownership transfers full ownership of the semen. This makes the recipient the legal owner of the semen and they will be the one to sign off on the litter registration when a litter is produced from the semen. Each Transfer (per stud) incurs a $89 transfer fee and their own annual storage fee ($103 vials or $122 straws). The new semen owner will then need to complete a Release Form to send or use the semen.
Authorization and Agreement for Semen Intake and Release for Artificial Insemination (AI)
To be used to authorize the intake, temporary storage and use of frozen semen for artificial inseminations at ICSB in Oregon.
Frozen Semen Release Form for In-Office AI's
Do not fill this out for shipping semen. This form is to be used to release and use frozen semen for TCI's at ICSB in Oregon.
Authorization for Vaginal Artificial Insemination
To be completed for Vaginal Artificial Inseminations (AI) at our office.
Frozen Semen Disposal Authorization Form
To be used to dispose of frozen canine semen stored at ICSB. This form must be signed by all owners of the semen.
An alternative is donating to either ICSB or AKC's Purebred Preservation Bank.
Semen Intake Authorization and Agreement Form
To be filled out and sent to ICSB ahead of frozen semen being shipped our office for temporary or permanent storage.
This is not the correct form for cryo kits or fresh-chilled sample to be frozen, see New Dog Authorization & Agreement Form or Repeat Authorization Form.
Transfer of Ownership Acceptance and Agreement Form
To be filled out by the recipient of a transfer of ownership of frozen canine semen stored at ICSB.
Account Reactivation Authorization and Agreement Form
To be filled out and sent in to ICSB when reactivating an account.
Updated Dog Information and Agreement Form
Updated agreement for the storage of a stud's semen stored here at ICSB in Oregon.
Post-Thaw Retest Authorization Form
To authorize the retest of the post-thaw motility of already frozen semen.
LN2 Shipping Tank Rental and Hold Harmless Agreement
To be filled out when ICSB sends a dry shipper/tank to pick up frozen semen at another facility. This requires pre-approval by ICSB, please contact us.
Authorization for TCI
To be used to authorize TCI's at ICSB in Oregon.
Stud Information Form for TCI
To be filled out by the stud or bitch owner for the stud's information for fresh-chilled or fresh-collected TCI's at ICSB in Oregon.
Authorization for Timing Services
To be completed for Progesterone Testing and/or Vaginal Cytology appts at our office.
Pregnancy Ultrasound Authorization Form
To be used for Pregnancy Ultrasound appointments.

Puppy Pak
A guide to the proper use and packing of the ICSB Puppy Pak. This kit is used to ship collected semen fresh-chilled from a stud owner to a bitch owner/veterinarian.
Cryo Kit
A guide to the proper use and packing of the ICSB Cryo Kit. This kit is used to send collected semen to ICSB fresh-chilled for subsequent cryogenic freezing. When this kit is returned to ICSB with a sample, a completed New Dog Authorization and Agreement Form or Repeat Authorization Form should be included.
Epi Kit
A guide to the proper use and packing of the ICSB Epi Kit. This kit is used to send testicles to ICSB for the extraction and cryogenic freezing of canine semen.
